Procedures for Admission

If the doctor determines that hospitalization is necessary, the patient will be admitted if he/she agrees after being provided an explanation to that effect. With the exception of emergencies, hospitalization is on an appointment basis. We will contact you in advance when a hospitalization date is determined.

When hospitalization is confirmed, please come to the reception counter on the first floor of Hokuto Hospital on the day of admission at the designated time.
Please complete the hospitalization request form and submit it to the reception counter.
Wheelchairs are available upon request. Please let us know should you require one.

What to bring on the day of hospitalization

  • Patient registration card and insurance card
  • Hospitalization request form
  • Inpatient medical history questionnaire
  • All currently prescribed medications
  • Certificate of qualification for publicly funded healthcare (healthcare for the elderly, infants, specified diseases, etc.)
  • Reduced payment approval certificates
  • Personal seal
  • Everyday items, footwear/slippers and other articles of daily use
    ※ Everyday items, T-belts and abdominal belts are available at the Hokuto Hospital Lawson store for your convenience.

What you cannot bring with you

Please do not bring the following items with you to the hospital.

  • Medicines, pets, alcoholic beverages and dangerous items such as knives.
  • Other things that can be a disturbance for other patients.

After the admission procedures are completed and explanation provided, our staff in charge will guide you to your hospital room. If you have any questions, please do not hesitate to ask.
