Outpatient Information

New patients

Reception

Please visit our reception counter for new patients.
After completing the consultation application form and the medical history questionnaire, our staff will accompany you to the central treatment room.
In the central treatment room, a nurse will confirm the medical history questionnaire, measure blood pressure and so forth.
If any tests are required, our staff will accompany you to the examination room.

Consultation

Our staff will accompany you to the consultation room.
If the doctor orders a test during the consultation, our staff will guide you to the examination room.
After the test, our staff will accompany you again

Appointment for your next visit

Patients who need an appointment for their next visit may request one at the reception counter for the appropriate department.
Then, our staff will take you to the payment counter.

Payment

Please make your payment at the cashier. The following credit cards are accepted: Visa, DC, MasterCard, JCB, American Express, Diners Club, Discover, UnionPay, NICOS, JACCS, NC, Saison, Nissenren and UC. Prescriptions for outside pharmacies are also provided at this counter.
Please provide your prescription and purchase your medication at an outside pharmacy.

Returning patients

Reception

Please visit the returning patient reception counter.
At Hokuto Clinic, automatic reception devices are available for your convenience.
Afterwards, please conduct a blood pressure self-measurement with the automated blood pressure monitor in the blood pressure measurement room.
If other tests are required, our staff will guide you to the examination room.

Consultation

Once you have completed the reception process, please wait in the lobby located in front of the consultation room for the department that you are visiting.
Our staff will call you when the doctor is available.
If the doctor orders a test during the consultation, our staff will guide you to the examination room.

Appointment for your next visit

Patients who need an appointment for their next visit may, may request one at the reception counter for the appropriate department.
Then, please make your way to the payment counter.

Payment

Please make your payment at the cashier. The following credit cards are accepted: Visa, DC, MasterCard, JCB, American Express, Diners Club, Discover, UnionPay, NICOS, JACCS, NC, Saison, Nissenren and UC.
Please provide your prescription and purchase your medicine at an outside pharmacy.

Appointment

Patients returning within three months from their last visit or who were told that they are eligible for an appointment may make an appointment or change the date or time of the appointment over the phone. Please note that in some circumstances we may not be able to make an appointment for you.

Monday through Friday: 14:00-17:00
(Hours may vary for some departments.)

Patients in need of emergency services Reception hours

Monday through Friday: 9:00-17:00
Saturday: 9:00-12:00
On Sundays or holidays and after reception hours, we will receive you at the night reception counter.
